Our intake process requires three major documents in order to begin therapy:
- The client contract
- Client intake form
- Notice of HIPAA practices
The notice of HIPAA practices applies to all of the therapy services that we offer. This document is to explain to our clients what their rights are as a patient. The client intake form provides us with some background information about you so that we can begin the therapy process quickly and smoothly. The client contract lets the client and therapist understand what is expected of both parties.
Each type of therapy has its own contract and intake forms.
